Animal Intake Form

Animal Haven is a small, limited-admissions shelter.

While we do accept owner relinquishments of cats and dogs from the public on a case-by-case basis, animals will be given a thorough behavioral and medical evaluation prior to admission.

  • We are often at or over capacity and cannot accommodate walk-in surrenders.
  • Appointments are required to surrender an animal to ensure we have space to accommodate.
  • Animal Haven reserves the right to refuse any animal.
